Child Forms

What we need from you:

1. Log into your Ravenna Student Management Portal here.

2. Start by updating your core information in My Account. This will include your email address, phone number and physical address.

3. Next, click on the Parent tab > click on the Enrollment Forms tab to access your back to school forms.

4. Click Fill out Form when you are ready to complete each form. 

5. This will take you to a screen where you will claim the form and start to fill it out. Only one parent/guardian can CLAIM each form and is then responsible for completing it. The exception is the Field Trip Liability Form, which will require all parents/guardians to log into their individual portal and sign. 

6. Each page may require you to fill out and/or approve that certain information is correct. You will be prompted with an error message to let you know what required information was not completed if, in fact, you do not complete the required information. We have also included instructions on certain pages to help guide you through this process. You will see several save options to Save and back to all forms, Save progress and go to the previous step, Save progress, or Save progress and go to the next step.

(We do not need updated forms from children who are graduating from Little School at the end of the summer.)

Please note:

• All parents must return their forms before their child can begin Summer Program. All but one of the enclosed forms can be completed by you. The physician’s form must be completed by your child’s doctor.
